Panchakarma (Bio purificatory treatments)

The ayurvedic approach to prevention and cure swings around two broad principls:

- The first is Nidan pariwarjana that is eliminating of the cause of the disease
- The second is Samprapti vighatana; that is, reversal of pathogenesis

In many early and reversible conditions, Sampratpti vighatana spontaneously follows nidan pariwarjana without any treatment. But in chronic diseases sameprapti vighatana may not occur even after nidan pariwarjana or even after palliative treatment procedures because vicious cycle is set and the system of the body learn to function in a pathological order and as such the pathology becomes the physiology of the altered state. Therefore, a chronic disease is to be treated in a comprehensive manner, it is important to find a real cure.

Ayurveda advocates twofold approach to cure:

1. Samsodhana (Biopurification)
2. Samsamana (Palliation)

Ayurveda propounds the doctrine of srotas, depicting that living body is made up of inimitable channels into which carry biological fluids, energies, and impulses can be applied as well as nutrients and medicaments used time to time for therapeutic purposes. These channels or srotas loose their integrity, competence, inherent functioning due to day to day physiological wear and tear; environmental pollution, stress, diseases, it is essential to restore the competence of the channels by specialised care. The classical idea of srotas system is comparable to the biomembrane system of body. Specialised care can help to improve functioning of the channels.

Ayurveda propounds the theory of Samsodhana, (that is, biopurification) and speciaficially describes the pachkarma therapy that is the five full therapy in order to tackle the issues of impurity and incompetence of channels (srotas), if the channels are washed and purified periodically by appropriate samchodhana (biopurification procedures) are used form time to time, one can easily prevent many diseases. Similarly if one attemps to purify the channels in morbid state one may successfully achieve the target of samprapathi vighatana (reverseval of pathogenesis) of a disease and a cure become possible.

Ayurvedic text describes Panchakarma procedures or five major karmas namely:

Vaman (vomiting therapy): endotoxine which are present in stomach cells and tissues of entire body are eliminated by Vaman karma.

Virechan (purgation therapy): endotexine are clear not only from the colon and rectum but also from full body that is from cellular level

Basti (colon cleansing): The process in which medicated oil or medicated decoction is administrated in to the rectum which detoxify the colon and other internal organs e.g. liver, pancreas, kidney etc

Nasya (errhine therapy): medicated oil is inhaled through the nostril, which is very useful in the diseases of upper part of the neck (such as, chronic sinusitis, chronic cold, chronic allergic rhinitis, insomnia, chronic headache migraine etc.)

Raktamokshana (blood letting): best therapy mentioned in ayurvedic classics for blood disorders, pittaj diseases and skin disorders eg.chronic eczema, psoriasis, non healing ulcers, vitiligo, alopecia areata, etc.

The Samsodhana therapy (biopurifying procedures) of ayurveda is an unique concept. It envisages not only visceral cleansing rather it aims at the total biopurification down to cellular level. If the body is biologically purfied and cleansed, the physiology is restored optimally and pathology reversed, the nutrients reach their desired destination easily and their bioavailability is enhanced.

Because of such fundamental benefits, panchakarma therapy has received a great scientific recognition in the present time.


VARIOUS PANCHKARMA TREATMENT THERAPIES :

1) SNEHANA THERAPY: Treatments are done using medicated oils / medicated ghrita which produces Sinigdhata (unctuousness), Mriduta (softness), Dravata (liquidity), Pichilata (sliney), in the body is named as Snehana or Oleation therapy. It is one of the important method of puravkarma (pre operative procedures) for Panchkarma treatments .

Adiministration of snehana is of two types :

A) Bahya snehana (external oleation)
B) Abhyantra snehana (internal oleation )


Different types of Bahaya snehana :

a) Abhyanga ( ayurvedic massage ) : it is the process by which the person’s body is oleated with medicated oils with specific pressures and movements. Abhyanga may differ according to pressure .

Characteristic of ayurvedic massage :

  • it enhances age and preserves longevity
  • it relieves the weakness
  • it strengthens the body vigor
  • it induces sleep
  • it increases the WBC (White Blood Cells) count and antibodies
  • increases immunity toward environmental changes
  • it gives you mental relaxation
  • it helps in balancing the vata dosha

b) Lepa : Application of a medicated paste on the body is known as lepa . Lepa is done for beauty treatments.and in skin disorders .

c) Samvahana: Massage which gives pleasure to the body and it should be done with the help of fingers. It acts as Aphrodisiac.

d) Udwartana: Massage in oppossing directions, twisting the body like a creeper . This massage is done in opposite direction of hair follicles with high pressure. It improves the skin is complexion.

e) Unmardana : Massage with a specific pressure and stoke on the body is known as unmardana .

f) Murdha Tail : Application of medicated oil to the head for a specific period is known as murdha tail .

Murdha Tail is classified into 4 types.

1) Shiro Abhyanga (head massage) : application of medicated oil to the head is said to be shiro abhyanga . Head is one of the important vital organ (marma), having all sense organs in it and hence one should protect it from stress and strain by doing shiro abhyanga.

2) Shirodhara : Pouring of fluids like decoction , medicated milk, medicated buttermilk, water , medicated curd medicated oils etc. over the head continuously for a specific period of time is known as shirodhara. Selection of fluids depends upon the disease and condition of the patient.

Properties of sirodhara :

  • it is very useful for headache, baldness, graying of hairs
  • it induces sleep
  • it is very useful in anxiety & tension headache
  • very useful in depression, psycological disorders
  • very usefull in nervous disorders
  • beneficial in hypertension
  • it alleviates the vatiated doshas

3) Shiro pichu : It is the application of a piece of a cotton or swab after dipping in medicated oil on the head (anterior fontanelle) or Brahmrandra eye .

Properties of Shiro pichu :

  • It is beneficial for all types of hair falls
  • Beneficial fo Facial paralysis
  • Burning sensation in eyes (netra stambha)
  • In high fever to control temperature one should apply ice or cold water Pichu.

4) Siro basti : It is a process by which the warm medicated oil is kept over the head for certain period with the help of leather cap.

Properties of Siro basti :

  • Facial paralysis. Insomnia
  • Dryness in the nose
  • Dryness in the mouth
  • Diseases of head
  • Eruption on the head .

g) Kati Basti : Kati basti is one of the varieties of Bahaya snehana. It is safe and give quick relief to the patient being an external therapy and has no side effect or complications. It gives immediate results because it acts at the relevent site of lesion and pacify the vata dosha .The process of keeping warm snehana (medicated oils or ghritas) on the lumber region with the help of mash pisti.

Indications :

  • low backache
  • protrusion of intervertebral disc
  • Sciatica
  • lumbar spondylitis

h) Greeva Basti : it gives quick result to patient because it acts at the site of the lesion and gives a soothing effect and nutrition to cartilage bone and improves the cushion action in between the two vertebral bodies. The process of keeping the warm medicated oils or ghritas on the cevical region with the help of mash pisti.

Indication :

  • Cervical pain
  • Intervertebral space reduction
  • Loss of lordosis in cervical vertebrae .
  • Chronic neck pain
  • Cervical spondylitis

I) Janu Basti : It is one of the varieties of bahaya snehana and is advised to the patient who is suffering from :

  • Osteoarthritis
  • Knee joint strain
  • Miniscus injuries
  • Ligament injuries

The process is to hold the warm medicated oils or ghrita on the knee joint with the help of mash pisti.

j) Akshi Tarpana : It is a process of keeping the Snehana (medicated oils / ghritas) in the eyes with the help of mash pisti (medicated paste) .

Indication :

  • Burning of eyes
  • Roughness in the eyes
  • Eye diseases caused by vata and pitta doshas
  • Difficulties in opening of eyes
  • Subconjunctival hemorrhage
  • Glaucoma
  • Corneal ulcer
  • Pain in eyes
  • Loss of secretion of lacrimal

k) Karna Poorna : It is a process in which warm medicated oils or ghee is poured into the ears .

Uses :

  • it prevents the occurrence of vata disorders
  • it relieves pain in the ears
  • it relieves ringing in the ears (Tinnitis )
  • it prevents neck stiffness and lock jaw .

l) Gandush : it is a process by which the medicated oil or medicated decoction should be held in the mouth for a certain period .

m) Kawala Graha : It is the process by which the medicated oil or medicated decoction can be easily and comfortably gargled in the mouth .

Uses :

  • It improves the voice and is beneficial for strength of the jaws
  • It improves the taste of food
  • It strengthens the gums and teeth
  • It relieves toothache
  • Reduces incidence of caries / cavaties in the teeth
  • Usefull in submucous fibrosis

B) Abhyantara snehana : (internal oleation)

It is a process in which Snehana ( medicated oils / ghritas ) is administered orally. Abhyantar snehana is one of the important therapy in ayurveda to treat various vata disorders as well as pitta and kapha disorders .


C) Swedana therapy : (herbal steam)

This is a process in which perspiration of the body is stimulated through a herbal steam bath.
